According to a University of Michigan Health Systems study conducted in 2008, African American women experience urinary incontinence less than half as often as Caucasian women. However, the study found that when incontinence does occur, the condition is usually worse than in white women. Black women experience a heavier flow of urine leakage during each occurrence of incontinence, sometimes to the point of noticeable wetting of their pants compared to only one third of white women.

The medical community has always believed that African American women don’t experience stress incontinence, or urine leakage during exercise, coughing, laughing or any activity that increases pressure in the abdominal area. Those with stress urinary incontinence have inadequate strength of the pelvic floor muscles. The U of M study proved otherwise. Black women do experience stress urinary incontinence, but may not have been diagnosed because of the common held belief that they don’t.

The study also found that African American women experience urge incontinence twice as often as Caucasian women. Urge incontinence, also known as overactive bladder, is characterized by the sudden contraction of the bladder’s muscular wall causing the urgency to use the bathroom and involuntary urine leakage. Fast movements often trigger urge incontinence and it can be acquired as a result of pregnancy.

In the past doctors also believed that the medical conditions that influence the occurrence of urinary incontinence are different between black and white women. The U of M study found that this is not the case. Those conditions, such as constipation, diabetes, obesity, chronic lung disease and depression were found to be the same in both races and cause urinary incontinence to occur at comparable rates between the two.

Numbers from the University of Michigan Health Systems study:
* The study included over twenty eight hundred African American and Caucasian women from three southeastern Michigan counties
* 27 percent of the women surveyed experienced moderate urinary incontinence
* Of these women experiencing moderate urinary incontinence, 14.6 percent were African American women and 33.1 percent were Caucasian women
* 25 percent of black women with moderate incontinence experience stress incontinence compared to 39 percent of white women
* 24 percent of black women with moderate incontinence experience urge incontinence compared to 11 percent of white women- 51 percent had a combination of both stress and urge incontinence
* The ages of women in the study ranged from 35 to 64 years old. The average age was 42 and around 70 percent had delivered at least one child vaginally

Overall, the study demonstrates that in the moderate category white women have significantly higher incidences of incontinence; black women may experience incontinence less often but at a higher severity. The study also showed that African American women have higher incidences of urge incontinence and that Caucasian women have higher incidences of stress incontinence.

Though there are many benefits of surrogacy, the cost of surrogacy is indeed not one of them. Intended parents pay dearly for the opportunity to hold their own baby in their arms, and the financial aspect of surrogacy unfortunately puts such a journey out of reach for many who face infertility.

A typical gestational surrogacy journey can cost between $50,000-100,000. Though some intended parents pay less than $50,000, and some pay more than $100,000, most will find themselves somewhere in this range, and should expect to pay this amount, even if they are lucky enough not to.

At first glance, it may seem as though the cost of surrogate mother are to blame for this outlay of funds by the intended parents, but the surrogate fee is a small amount of that total dollar figure. There are many other factors that need to be considered when examining the cost of surrogacy.

First, there are the legal fees. Every surrogacy arrangement will need to have at least two lawyers involved. The intended parents will need an attorney to draw up their contract, represent them throughout the journey, and get the birth certificate and various other legalities taken care of for the baby’s birth.

Intended parents who pursue surrogacy are not necessarily the rich and privileged. Instead, they are most likely a set of hard-working, middle class people who are driven to making their ultimate dream come true: their family.

Many sacrifice things such as savings accounts, new cars, vacations, bigger homes or go into debt with a home equity loan or credit cards in order to make the dream of surrogacy a reality. And hopefully, when this experience is over, they take home with them a new baby, and the beginning of their future.

Imagine you are a male and you see your physician for your yearly checkup. Imagine the doctor orders blood tests, including a PSA test for the early detection of prostate cancer. So far, so good. This is the way to check whether a man without any symptoms of prostate cancer may in fact have it. Imagine the tests came back outside the normal range

Yet, there is still debate amid some doctors over the way to screen asymptomatic male patients for prostate cancer or whether to screen at all. They hold that screening has little, if any, value. If of a screening test is abnormal the patient needs to be informed about the results and either be referred to a specialist or be told about the option for diagnostic testing, like a biopsy. Once more, however, some doctors also believe that, at least under certain circumstances, a man diagnosed with prostate cancer does not have to undergo treatment right away and simply needs to carefully monitor the cancer.

If the doctor does not give the patient the option to undergo screening or does not tell the patient about the abnormal test results the patient’s prostate cancer may spread and metastasize without the man even knowing he may have cancer. Unfortunately, if a physician noted that the patient’s prostate was enlarged or there was a nodule on the gland and the PSA test results suggested abnormally high levels of the antigen and the physician did not notify the man of the abnormal results, the individual would in all likelihood assume that meant there was no need to follow up. If the patient does actually have cancer, not informing the patient that he may have cancer will postpone his diagnosis.

A delay might, in turn, give the cancer time to reach an advanced stage. Once a cancer metastasizes treatment will at best delay the progression of the cancer and lower the effects (like pain) of the cancer. Under such circumstances, that patient and his family might be able to pursue a failure to diagnose medical malpractice lawsuit against the physician.

Screening tests may have false positives. This means that certain patients with abnormal screening results will not have cancer. But doing screening tests for cancer is meaningless if there is no follow up as it gives the patient a false sense of security thinking that he has no cancer as the doctor tested him and said nothing to him that the tests demonstrated he might have cancer. Doctors generally concur that there is a need for follow up if the results of screening tests come back as abnormal.
